Our practice's area is an almost circular patch which lies between Canterbury Road, Purley Way, Duppas Hill Road, North End Croydon, Wellesley Road, part of St James' Road and Windmill Road, St Saviour's Road, Campbell Road and Cameron Road.

If your registered address does not lie within this catchment area you may not be able to register with this practice and will have to find another GP closer to you. However please speak to reception staff who will be able to help you.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice. In addition, please bring some photographic ID and proof of address eg utility bill, tenancy agreement, bank statement. The proof of address MUST be dated within the last 3 months. If you are on a visa, you will be asked to show this as well. Only once your registration has been approved and accepted will you be able to make an appointment to see a GP or Nurse.
